Electronic Forms


Starting Friday, March 8, 2019, the Graduate Affairs will be releasing 8 graduate forms electronically.  These forms are Change of Status, Petition for Adjustment of Academic Requirements (PAAR), Withdrawal or Dismissal, Petition to Repeat WU Graded Courses, Validation for Recency, Appeal for Time Limit Extension, Request for Letter of Completion and Report of Final Exam or Thesis Defense.

This does not include petitions that need to go to the Registrar (such as Late Schedule Adjustment) and Thesis Committee form.

To access the electronic forms follow the steps below:

  • Go to the Graduate Affairs site https://grad.sdsu.edu/ and click on Current Graduate Students
  • Click on Forms
  • Scroll all the way down to E-Forms
  • Click on the form you want to access
  • Log in using your SDSUID, and the form self-populates with their information

Once the form is submitted, the Graduate Advisor will be notified.
